BestCare IPA is required by law to maintain the privacy of your protected health information (PHI), give you this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect.

We use and disclose PHI for treatment, payment, and health care operations, and for other purposes permitted or required by law. We will obtain your written authorization for any other use or disclosure.
Your rights
You have the right to inspect and copy your PHI, request amendments, request an accounting of disclosures, request restrictions on certain uses or disclosures, request confidential communications, and receive a paper copy of this notice.
